Comments:
F/O RW Kostiuk was a pilot with 444 Squadron at 4 (F) Wing, Baden Germany. He was killed when his aircraft (Sabre #23120) rolled in while turning final on approach to land. The subsequent investigation indicated a suspected right aileron hinge failure.
